我从事CRA的一点体会
一年前我已从事临床工作近五年,考了中级职称后改的行。个中原因主要感到从事医生使我顾家太少,尽管我很热爱这份工作。
医生是个造福后代的职业,自己得到和付出的即便在升到主治后可能略相称,可也没有时间安排妥自己的生活。
一年前加入了家CRO,进公司后我先后参与或负责或参与协助近5个项目。其中有III期的,IV期的,还有上市后的调研。Sponsor都是世界药企前十的几家。有的项目从选中心开始,组织研究者会议,递交伦理,等待审批,参与中心启动,培训医生填写CRF,原始数据核查,解决Query,随时报告不良事件。有的是去清点回收药品,需要花费大量的时间,很耐心,很细致。有的是去做SDV,即Source Documents Verification,原始文件核对。有的是去回收CRF,然后整理齐备交给数据统计部门。有的就是纯粹后期的解决一个又一个的Query。有的有不少不良事件报告需要Follow。
目前国内医疗行业的大环境不好,导致很多同行和学弟学妹都渴望从事CRA(Clinical Research Associate)。
我认为在没有充分了解这个行业之前,还是脚踏实地在目前的岗位上兢兢业业,不要一山望着一山高,这样患得患失反而会进入一种频繁跳槽却总也感觉找不到最合适自己的怪圈,最后把心态也搞坏了。尤其是三十岁以前,是一个学习、积累的过程,其中包括知识、经验和人脉。
我进入CRA行业前准备了三年多的时间来了解、参与、实践。
首先,CRA不仅需要良好的医学背景还需要对整个医疗运作体制的充分掌握,因此很多应届毕业生直接进入CRA行业很可能无法理解为什么会有各种各样层次的教授主任,毕竟在实习阶段没有任何利益关系,就无法真正地融入医院的内部人文环境中。
其次,就是对CRA这份职业的了解。起初我以为多看国家GCP,国际ICH-GCP就够了,其实完全不是这样。面试的时候这些最多作为笔试的一些基本常识,此外更重要地是真枪实弹地实践积累下来后处理应急事件的经验。
最后一点,应该也是最重要的,就是英语,知名的CRO或者药企中的CRA必须按照项目对应的SOP书写很多英文报告,每天工作中可能需要收发几十封不同项目来自不同角色的Email,乃至在国际多中心项目时的电话会议都要求良好的英文听说读写能力,在这一点上如果英语基础好的人在面试时也会获得比较高的印象分,这点随着国际化接轨的加快,在其他行业也是显而易见的。
之前看到一个战友写的对CRA的一些看法,我感到写得非常中肯,我加入一些我个人体会,修改如下,还望战友不要见怪,旨在交流:
1.CRA前景:CRA 是个热门的.职业;国内的起步比较晚,中国的人口数量多,SFDA的标准有改变,外企重视中国的市场等,所以有比较大的发展前景。在外资药企内做3-5年就算是资深的CRA; 一般在CRO 公司(正规的,做多国多中心试验)做了3年都是资深的CRA。
2.CRA薪水:CRA 不是你想象的天堂,赚的绝对是血汗钱。在大的药企或CRO,工作非常辛苦,而且强度很大,压力也很大;尽管出差多数是出去一两天,不占用周末和假期,但是timeline压得根本没时间出去,每天都是酒店和医院两头跑。绝对会感觉对得起你拿的那些薪水;CRO 新人:薪水在5000左右(税前)。大型药企多数有年底Bonus,新人:薪水:7000~9000(税前);有3年或以上经验的CRA,去了大型外资药企,平均的薪水:10000(税前);待升到Senior CRA通常正规的CRO:6000~8000(税前),药企的高级CRA也差不多会多2000左右,但是药企中很多CRA同时管项目,可以是CRA也可能SCRA,资深的CRA,可达到:10000~12000(税前);到项目经理即Project Manager(在药企没有这个Title),在CRO通常可能需要入行1年到3年,在药企很可能需要3~5年,薪水都差不多,大约:12000~15000(税前);再上升到临床研究经理即CRM及高级CRM,CRO里大约15000~18000(税前),大型药企分工比较细,会细分到事务、培训、产品安全、产品注册、产品医生或医学总监等岗位,薪水跟工作量以及公司本身都有些关系。特别指出的是:项目经理之后有一些公司除了Salary就会有项目提成,年底也会有额外的Bonus,还会有相当不错的Well-Being,除了Social Benefit还有各自企业不通的Welfare Plan。
3.CRA发展的空间和曲线: 国外的不少CRA会在同一家公司一直做到退休,而国内相对就非常少了,然而有一点跟医生很相似,就是也有一条很固定的职业发展轨迹,是个很讲究专业的行业,需要不断提升自己在该领域的业务技能与常识,此外对临床前沿的东西也最好随时可以拿起,这样跟那些知名教授主任讨论项目的时候他们才会尊重CRA,也会非常配合保质保量地完成项目; 一般的发展模式是:Junior CRA-Senior CRA-[Project
Leader(PL)、Project Manager(PM)]-Associate manager-Associate director-Clinical Research Manager-Medical Department Manager;如果往BD(Business Development)或Trainer方向发展也可以,如今很多的PM在管理项目的时候就会参与到BD的工作中去,但还是要根据不同公司的职责划分来定。
4.CRA 的适合人群
第一,CRA无论在药企的医学部还是CRO的操作部都是参与到项目的每个环节,在项目形成雏形的时候像个策划,制定方案的时候像个研究生,翻译方案和知情同意书的时候是个翻译,查阅文献修改方案通过律师考核的过程像个督查,成本核算规划预算的时候像个会计,选医院选医生的时候像个猎头,进入医院考察接触教授主任的时候像在做课题,递交伦理的时候如同个秘书,组织研究者会议的时候像做市场和大型会议的组织人员,培训医院医生启动项目的时候像个培训专员,招募受试者的时候又如同一个销售,然而核对数据清点材料和药品的时候又好像个公务员,监查的过程要非常细致如同个苛刻的审计或生物统计,发生严重不良事件时又像个临床医生,在跟踪不良事件报告的过程中如同个安全专员,结束项目总结报告整理材料的时候又像个文员,总之亲力亲为什么都要做;跟各种各样的人打交道,交际能力要没有问题。
第二,CRA 要负责的事务会非常繁琐并要求极度严谨,你会感到几近吹毛求疵的程度,毫不亚于临床对病例书写的规范要求。很多人做了CRA 后,感觉很有种挫折感,经常会出纰漏,但又不一定是故意的,只是事情太多应付不过来。认真的工作态度就可能轻而易举地处理好这些难题。
第三,CRA会经常出差,如果负责的Site(医院)多一些,一年中很可能有三分之二的时间在外面,因此要合理安排好工作与生活,毕竟出差时间只是一两天,不影响任何休息日,即便不得以影响了也会调休。但出差时更要工作细致,一旦有纰漏会增加成本。此外需要清醒的头脑去处理很多突发事件,尤其在外地,Site的管理远没有在本地城市的医院容易,时间都是按照小时计算好的,如果效率低或者不仔细就容易给自己添麻烦。
5.CRA工作的现状:由于国内跟国外临床试验管理和监督方面还有相当大的差距,所以如今入行CRA会经常感到许多医生还缺乏基本素质,往往大城市三甲医院从事较多国际多中心临床试验的教授和主任在这些方面做得已经很不错了,可以看到他们会准时在办公室等候CRA,把事先电话里确定下来的文件准备好给CRA看,如果有任何问题会给予耐心的解释甚至会经过CRA指正后谦和地分析并商榷解决方法;然而,还存在不少医生被以往不良氛围影响下养成了某种惯性思维方式,或在巨大的生活和工作压力之下,不理解临床试验的真实目的及意义,对GCP也知之甚少,这样在接触中就要求CRA懂得如何去解决这些人为因素造成的阻碍,把不良因素降低到最低限度。具备比较完善的SOP的一些外资CRO或药企医学部通常还是有比较浓郁的学术氛围,可以在从事CRA的经历中获得在医院永远无法获取的人生阅历。
